NBI Terms Update

On February 1, 2006 NBI revised the terms which under which you participate in our Locator Program.

Changes include:
* The referral fee for each measure set at $5 (was $0)
* The referral fee for each blind set at $6 (was $5)

Please review the revised terms below, and indicate your acceptance by checking the box and clicking [OK].  You will then return to job page #101,999.
